I am a UK tax payer and would like King George and Queen's Hospitals Charity to claim back the tax on all donations I have made for the four years prior to this year and all donations I make in future, unless I notify you otherwise.^†

^† Please note you must pay an amount of income tax and/or capital gains tax for each tax year (6 April one year to 5 April the next) that is at least equal to the amount of tax that all the charities and community amateur sports clubs (CASCs) that you donate to will reclaim on your donations for that tax year. All other taxes such as VAT and council tax do not qualify. We will reclaim 25p of the tax on every £1 that you have given. Please let us know if your circumstances change.
